The Team
Join our Community, Mental Health & Wellbeing team as a Clinical Nurse Specialist with a focus on Discharge Planning, based at the Millbrook Rise Centre.
This position offers the chance to make a meaningful difference in improving the quality of life for our Mental Health Consumers and successfully setting them up in the community to live long-term.
Millbrook Rise Centre is a 27-bed mental health subacute inpatient facility; we are a consumer-centred service with a focus on rehabilitation and recovery located in the beautiful Derwent Valley. We work closely with various services within not only Statewide Mental Health Services (SMHS) but also with the wider community services such as NDIS providers.
We are a collaborative team that works with our consumers, their friends and families in their individualised recovery journey. We have a constantly developing activities and therapeutic groups program and we welcome new staff and new ideas to contribute to the activities program at the site.
The Role
The Clinical Nurse Specialist - Discharge Planning will undertake the coordination of quality discharge planning for patients with severe and enduring complex mental health needs, developing change management strategies to facilitate timely access and streamlined patient flow.
You will need
- Demonstrated ability to deliver high quality mental health care in an acute inpatient mental health setting with a particular focus on discharge planning assessments, risk assessments, brief psychological interventions and developing management plans.
- Extensive demonstrated experience working in an inpatient mental health setting in an autonomous role and as part of a multidisciplinary team.
- Demonstrated knowledge of the psychological and healthcare needs of individuals, including the understanding of recovery principles, policies and procedures, and associated legislation which impact upon inpatient and community based settings.

Details of appointment
Permanent part time, day worker position, working 60 hours per fortnight, commencing as soon as possible.
*notwithstanding hours to be negotiated with the successful applicant.

Salary
$109,767 to $115,813 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
- Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
In addition, the following allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10% based on qualification type
