Exciting opportunity for a Registered Nurse to join the Department of Health!

The Team:

Care@home is a state‑wide interdisciplinary virtual care service that supports Tasmanians to safely manage acute and chronic health conditions from the comfort of their own home. Using innovative virtual monitoring technology, the team remotely assesses, coaches, and coordinates care for eligible patients, helping them stay well in their preferred environment while reducing potentially preventable hospital admissions and facilitate early discharges from hospital.

The Role:

As a Registered Nurse within the Care@home service, you will play a key role in delivering high‑quality remote nursing care using telehealth and virtual monitoring tools. Your responsibilities will include:

  • Conducting remote assessments of patients’ symptoms, wellbeing, and clinical needs.
  • Monitoring acute or chronic illnesses through virtual care technology and responding to changes in patient condition.
  • Providing evidence‑based nursing advice, education, and health coaching to support patient self‑management.
  • Coordinating care and collaborating with multidisciplinary team members, including medical officers and allied health staff, to ensure safe and appropriate care in the home setting.

This is a dynamic and progressive nursing position where you’ll be part of an innovative model of care at the forefront of virtual health delivery in Tasmania.

You will need:

  1. Sound interpersonal and communication skills, including written skills and the ability to function effectively in a multidisciplinary team environment.
  2. Knowledge of continuous quality improvement (safety and quality) and the application of evidence based practice in the practice setting.
  3. Ability to undertake patient/client education in the practice setting, together with a commitment to participate in ongoing professional development.

Details of appointment:

Multiple permanent full time or part time, shift worker position, working up to 76 hours per fortnight, commencing as soon as possible. 

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $80,524 to $109,175 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

Access to the Grade 4 salary range $102,439- $109,175 is subject to successful application for progression to Grade 4

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type
