Job description:

Support high‑quality aged care by assessing needs, coordinating services, and working with older Tasmanians to promote wellbeing and independence.

The Team:

The Team within the Department of Health Tasmania works collaboratively to support high‑quality aged care services in their roles as Aged Care Assessors (Non‑Clinical). The team plays an important role in assessing individual needs, coordinating services, and supporting informed decision‑making to enhance the wellbeing and independence of older Tasmanians. Working closely with clients, families, service providers, and stakeholders, the team is committed to person‑centred care, clear communication, and positive outcomes for the community.

The Role:

Provide an Aged Care Assessment (non-clinical) to undertake Simple Home Support Assessments utilising the My Aged Care platform for older people over 65 years of age.  Assessments are completed using the Integrated Assessment Tool (IAT) in a timely manner to assesses an older person’s needs, goals and preferences on wellness and promoting independence.  Including assessing for eligibility for Commonwealth Home Support Program (CHSP) services.  The Aged Care Assessor works with the older person to assess their needs, goals and preferences focusing on wellness and reablement approach to build their independence.  

You will need to:

  • Working autonomously and as a member of a broader Aged Care Assessment service, deliver key performance indicators within identified timeframes.
  • Using a service provider portal and other websites, to support clients access services. 
  • Complete face to face Home Support Assessments using the Integrated Assessment Tool (IAT) to assess eligibility for aged care programs and services. Provide a holistic assessment and goal orientated wellness approach to client assessments and client phone/face to face reviews.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*Notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $81,915 to $94,263 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Public Sector Unions Wages Agreement 2022.

Eligibility:

  • Current Driver’s Licence
  • Previous experience in case management, aged care or support work 
  • care co-ordination and individualised service planning.
