Job description:

Undertaking comprehensive geriatric assessment, identifying early discharge issues to enable the early implementation of discharge planning processes for complex patients and referral to appropriate disciplines to assist as required. Acting as resource for clinical staff in relation to changed behaviours associated with a delirium or other cognitive impairments such as a dementia. Undertaking specific program activities for AST to ensure quality and safe clinical care processes of elderly patients, under broad direction of the Nurse Unit Manager

The role:

In association with the hospital home teams and Aged Services, assist in ensuring the provision of integrated care between acute and sub/non-acute sectors by:

  • Undertaking comprehensive geriatric assessment, identifying early discharge issues to enable the early implementation of discharge planning processes for complex patients and referral to appropriate disciplines to assist as required.
  • Acting as resource for clinical staff in relation to changed behaviours associated with a delirium or other cognitive impairments such as a dementia.
  • Undertaking specific program activities for AST to ensure quality and safe clinical care processes of elderly patients, under broad direction of the Nurse Unit Manager

Details of Appointment 

Fixed term, part time, day worker position working 64 hours per fortnight, commencing as soon as possible, for a period of approximately nine (9) months. 

Permanent, part time, day worker position working 32 hours per fortnight, commencing as soon as possible.

*notwithstanding hours may be negotiated with the successful applicant

  • Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.

Salary: $75,902 - $102,908 (pro-rata) per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

In addition, the following allowances calculated on the salaried incremental point may be available: 

  • Professional Development Allowance of up to $500 per annum
  • Access to the Grade 4 salary range $96,558 - $102,908 is subject to successful application for progression to Grade 4
  • Post Graduate Allowance of up to 10% based on qualification type
  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility:

Successful applicants will be required to meet the essential criteria:

  1. Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

  1. Current Driver’s Licence

  2. Proven experience in the comprehensive assessment and management of clients with aged care related issues

  3. Experience with managing clients with delirium and/or dementia

The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted:
