Job description:

In this role, you will provide care collaboratively with the patient/client and carer, work in collaboration with local providers and assist in coordination of services and service department and other members of the multidisciplinary team as relevant.

The Team:

Care@home is a state-wide interdisciplinary service that supports patients to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions.

This virtual care service has been established as a state-wide navigation hub, that provides a single point of access for home and community-based care; making it easier for both consumers and clinicians to identify what care pathways are available to support people in their preferred environment.

The Care@home team includes general practitioners, nurses and allied health clinicians including pharmacy and social work.

VMOS (Virtual Multidisciplinary Outreach Service) is a new service arm of Care@home supported by the Commonwealth Strengthening Medicare – Supporting Older Australians initiative. The service will deliver timely multidisciplinary assessment, care planning, and escalation support to people living in Residential Aged Care Homes (RACH) in Tasmania who present with complex needs, with the objectives of improving quality of care, supporting RACH capability, and reducing avoidable hospital presentations and admissions.  

The Role:

  • Provide and coordinate a dedicated allied health service for the management of patients/clients within the complex virtual care programs, with a focus on VMOS.
  • Assist, plan and arrange implementation of specific treatment programs for individual clients according to current evidence and best practice guidelines within a collaborative and multidisciplinary framework and in accordance with national standards, Agency directions, policies, legal requirements, and professional competencies.
  • Work collaboratively, in an allied health role, with other members of the multidisciplinary team and other agencies to facilitate timely interventions and smooth patient flow between services.
  • Supervise, educate and assess the competency of allied health professionals, undergraduate allied health professionals of their own discipline, other health professionals and relevant support staff as required.
  • Assist in building clinical workforce awareness, knowledge and capacity to facilitate appropriate responses related to complex virtual care.
  • Assist in the provision of appropriate complex virtual care services and resources for people from a range of backgrounds including culturally and linguistically diverse backgrounds and Tasmanian Aboriginal people and communities.

You will need:

  • Tertiary qualification in an allied health discipline
  • Demonstrated relevant clinical experience as an Allied Health Professional with knowledge of evidence-based practice within a complex health care setting and/ or virtual patient/client care and management.
  • Demonstrated high-level communication skills (both written and verbal) and interpersonal skills to liaise with a broad range of internal and external stakeholders, as well as conflict resolution skills and the ability to engage in difficult conversations.
  • Proven commitment to evidence-based practice activities, quality improvement, policy development and research with the ability to analyse service needs and plan, implement and evaluate services accordingly.
  • Demonstrated ability to develop and maximise productive working relationships with a range of health professionals, organisations and service providers within a complex health system and strict legislative framework.
  • Demonstrated capacity to be adaptable and responsive to change. Proven ability to be proactive and self-directed in an environment subject to high pressure and change.
  • Proven ability to work effectively within a small multidisciplinary team, including the ability to cooperate and work well with others, respect the needs and ideas of others, and provide support and assistance to team members.

Details of appointment:

Fixed term full time, day worker position, working 76 hours per fortnight, commencing as soon as possible until 30 June 2028. 

*Notwithstanding hours to be negotiated with the successful applicant.

Salary: $110,425 to $121,907 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement No. 2 of 2022.

Salary Packaging:

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible. 

Allowances: 

  • Please note that access to salaries beyond $119,290 is subject to qualifications and/or application to the personal upgrade scheme.

In addition, the following allowances calculated on the salaried incremental point may be available: 

  • Professional Development Allowance of up to $1000 per annum

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Satisfactory completion of an approved allied health professional tertiary qualification/program of study and registered with the relevant National Board or in the case of self-regulated allied health professions with either full membership or eligible for membership with the relevant professional association.

*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.
