The Team:
The NWRH Social Workers team practices collaboratively with the wider multidiscipline team to support:
- Vulnerable people experiencing an acute medical event and likely to require lifestyle adjustment.
- Inpatient discharge planning.
- Identified social care needs during and post their hospitalisation.
Reporting to the manager of Social Work services our dynamic and supportive team comprise of hospital-based Social Workers providing psychosocial support to inpatients and outpatients at both hospital sites and our community-based Social Workers supporting hospital avoidance, wellness and reablement, and groupwork support to clients through Primary Health funded services. Our team proactively engages with the multi discipline team and external agencies to ensure best outcomes for our Patients, we practice collaboratively within the Allied Health team across our acute and community services and promote a culture of shared learning and respect for diverse perspectives.
The Role:
As a member of the Hospitals North/North West Social Work team, the incumbent will, as part of a multidisciplinary team, contribute to the development, provision and evaluation of social work services for patients of the Hospital and their families and/or the community; in accordance with the Code of Ethics of the Australian Association of Social Workers and the Code of Conduct of the Hospitals North/North West.
The successful candidate will:
- Have the opportunity to practice across all other clinical caseloads including Medicine, Surgery and Emergency.
- Receive ongoing support and supervision provided by a Senior Social Worker.
- Be part of a team built on collaboration, trust, respect and teamwork
Opportunity for rotation across other acute clinical caseloads i.e. medical, surgical, and emergency departments may also be provided when workload demand indicates the need.
Please note - the successful applicant will be required to participate in a weekend on-call Social Work Service.

Details of appointment:
Fixed term, full time, day worker position, working 76 hours per fortnight, from 3 November 2025 until 27 November 2026.
*notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $73,834 to $111,364 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
- Salary range is in accordance with Allied Health Professionals Public Sector Union Wages Agreement No. 2 of 2022

Hold, or be eligible for, full membership with the Australian Association of Social Workers (AASW)

Current Working with Children Registration.
