Job description:

Carry out assessments, plan and deliver a range of social work interventions for clients and their carers/families referred to the Community Rehabilitation Unit (CRU) in accordance with organisational policies and the professional code of conduct.

The Role:

Within the Tasmanian Health Service Framework and in accordance with Agency policy and procedures, the social worker will:

  • Carry out assessments, plan and deliver a range of social work interventions for clients and their carers/families referred to the Community Rehabilitation Unit (CRU) in accordance with organisational policies and the professional code of conduct.
  • Provide care collaboratively with other members of the interdisciplinary team.
  • Accurately record and maintain unit data requirements, case notes and discharge summaries to the required professional standard.
  • Positively engage in clinical social work supervision and pro-actively maintain contemporary knowledge through appropriate continuing professional development.
  • Assist the Senior Social Worker in ensuring best practice standards for social work services provided at CRU.

The CRU Service and Team:

  • Deliver adult outpatient rehabilitation service working with clients with a diverse range of diagnostic conditions, primarily acquired neurological conditions (e.g. stroke, traumatic brain injury) and progressive neurological conditions (e.g. Parkinson’s disease, Multiple Sclerosis).
  • Comprise an interdisciplinary team (including Occupational Therapy, Physiotherapy, Psychology, Dietetics, Social Work, Rehabilitation Nursing, Art Tutor, Allied Health Assistants) with a focus on client centred goal setting and rehabilitation planning.
  • Work with clients to achieve their rehabilitation goals in our well-equipped centre and in their home and community.

You will need:

  • Knowledge of current contemporary social work and client-centred practice including the clinical management of clients with complex care needs relevant to an adult rehabilitation context.
  • Demonstrated ability to exercise professional judgement and initiative and to work with general supervision and to seek guidance as appropriate.
  • Demonstrated time management skills and ability to prioritise and monitor a busy workload while providing a high standard of client care.

Details of appointment:

Permanent part time, day worker position, working 68.4 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $73,834 to $111,364 per annum pro rata. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement No. 2 of 2022

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers.

It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change.
