Job description:

The Hospital@home: South service plays a crucial role in the communities of Southern Tasmania. It provides hospital avoidance interventions and acute and sub-acute care for adults, ensuring safe and effective multidisciplinary hospital-level care in the community.

The Hospital@home South service plays a crucial role in the communities of Southern Tasmania. It provides hospital avoidance interventions and acute and sub-acute care for adults, ensuring safe and effective multidisciplinary hospital-level care in the community.

The service operates across multiple sites throughout Southern Tasmania. Hospital@home: South satellite sites are currently located in Cambridge, Glenorchy, and Risdon Vale. 

The Role:

Associate Nurse Unit Manager: Geriatric Evaluation and Management at Home (GEM@home) - Hospital@home

  • Works within a multidisciplinary team, and with individuals and their families/carers to support the complex care needs of those admitted to the GEM@home service.
  • Triage, assessment, coordination and communication of referrals with internal and external stakeholders.
  • Nurse liaison and leadership for the GEM@home service.
  • Duties as per the Associate Nurse Unit Manager (ANUM) Statement of Duties.

You will need:

  • Demonstrated time management and organisational skills, including the ability to prioritise clinical and administrative tasks within a busy environment subject to work pressure and change.
  • Proven commitment to teamwork, together with advanced interpersonal skills, including written and verbal communication, and the ability to lead, manage conflict and work effectively with a multidisciplinary team in the planning and coordination of clinical care for patients.
  • Demonstrated advanced understanding of clinical risk and its implications for practice, including the application of Safety and Quality and Work Health and Safety legislation across the clinical setting, with the ability to develop a culture of risk mitigation.

Details of appointment:

Permanent, part time, shift worker (fully rotational) position, working 70 hours per fortnight, commencing as soon as possible. 

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
