Job description:

Our aim is to support young people (16 to 24 years old) who may be experiencing an acute episode of mental illness to receive treatment in their home and within the community, rather than being admitted to an inpatient mental health unit, or as part of a discharge plan from hospital.

** The Department of Health is seeking suitably qualified applicants for a Clinical Nurse Specialist - Y-HiTH (528053) vacancy.

The Team:

The Youth Hospital in the Home (Y-HiTH) service, part of the Child and Youth Mental Health Service (CYMHS) in the North-West region of Tasmania, is a vital initiative for several reasons:

  • Bridges a Critical Service Gap: As a new service based in Devonport, Y-HiTH addresses a previously unmet need for intensive, home-based mental health care for young people in the North-West. This region has historically faced challenges in accessing timely and appropriate mental health support.

  • Reduces Hospital Admissions: By delivering acute care in the home, Y-HiTH helps prevent unnecessary hospitalisations, allowing young people to receive high-quality treatment in a less disruptive and more therapeutic environment.

  • Supports Family-Centred Recovery: The home-based model enables families to be more actively involved in the care process, fostering stronger support networks and improving long-term outcomes for young people experiencing mental health crises.

Then come and join our new Youth Mental Health Hospital in the Home (Y-HiTH) service!

The Y-HiTH team will work with the young person and their family/significant others to develop an individual treatment plan that supports their recovery. With multiple home visits occurring everyday and additional support via telephone and telehealth, young people will receive comparable treatment from Y-HiTH as they would from an inpatient admission.

The Role:

The Child and Youth Mental Health Service (CYMHS) Youth Hospital in The Home (Y-HiTH) is a new service for the North-West region, based in Devonport.

In this clinical role, and under the guidance of the Y-HiTH Nurse Unit Manager, you will work collaboratively within a multidisciplinary team to deliver care in accordance with an agreed care plan

You will need

  • Clinical experience in child and youth mental health, with the ability to deliver acute, recovery-oriented care in a home or community setting.
  • Strong communication and collaboration skills to work effectively within a multidisciplinary team and engage with young people, families, and community supports.
  • Capacity to work autonomously in a dynamic environment, including the ability to manage risk, respond to crises, and adapt care plans in real time.

Details of appointment:

Permanent full time, shift worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $106,570 to $112,440 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available: 

  • Post Graduate Allowance of up to 10 % based on qualification type

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
  • Current Working with Children Registration
  • Current Driver’s License
