Job description:

Exciting opportunity for a Clinical Nurse Consultant to join the Department of Health!

The Team:

The Department of Health is seeking experienced and dedicated Clinical Nurse Consultants to join our team as Residential Aged Care Home (RACH) Liaison Nurses. These roles are pivotal in ensuring seamless transitions for patients moving between hospital and residential aged care homes, providing expert clinical advice, and fostering strong partnerships with aged care providers.

Exciting opportunity for RACH Liaison Nurses in Tasmania's hospitals, working with Hospitals South, North or North West Intergrated Operations Centres (IOC) and Hospital Aged Care Liaison Teams (HALT).

The Role:

Work in partnership with IOC or HALT Leadership Teams, clinical staff, and Private Providers to assist patients and their families navigate the pathway from hospital to a RACH. 

  • Provide expert clinical leadership and advice within an interdisciplinary team.
  • Conduct comprehensive patient assessments and coordinate care plans for transfers to RACHs.
  • Collaborate with RACH providers and General Practitioners to ensure continuity of care.
  • Utilise and evaluate models of care and service delivery based on evidence-based practices.

Details of appointment:

Multiple fixed term, full time, shift worker position, working up to 76 hours per fortnight, until 30 June 2028.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $114,851 to $121,866 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available: 

  • Post Graduate Allowance of up to 10 % based on qualification type 

Salary Packaging :

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible. 

Eligibility:

Successful applicants will be required to meet the essential criteria 

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

  • Holds or is working towards a relevant post graduate tertiary qualification and/or recent extensive nursing experience relevant to the role.
