Exciting opportunity for a Nurse Manager to join the Department of Health!

The Team:

The Care@home team is part of the Department of Health (DOH) and is dedicated to enhancing the health and well-being of the Tasmanian community. This state-wide interdisciplinary service remotely supports patients in managing their illnesses and chronic conditions from their homes using virtual care technologies. The team collaborates closely with various healthcare providers, to deliver excellence in healthcare, achieving improved outcomes for consumers, the workforce, and the broader Tasmanian community

The Role:

Nurse Unit Manager -Care@Home

  • Provides leadership for the Care@Home program.
  • Ensures the efficient and effective provision of care, based on clinical standards and best practice principles within a collaborative and multidisciplinary framework by coordinating the clinical, management, education, and nursing research functions within the Care@Home program.
  • Provides the Nursing Director - Care@Home program with high level information and advice on issues relating to clinical operations and management of physical and human resources within the Care@Home program.

Key Responsibilities:

  • Leadership and Management: Oversee the daily operations of the Care@home service, ensuring high-quality care delivery and efficient use of resources.
  • Patient Care Coordination: Manage the Acute Virtual Monitoring Program (AVMP) and Chronic Disease Management Program (CDMP), providing support to patients with acute and chronic conditions.
  • Team Collaboration: Work closely with an Interdisciplinary team, including General Practitioners, to ensure comprehensive patient care.
  • Virtual Care Implementation: Utilise virtual care technologies to support patients in their homes or preferred place of residence, improving health outcomes and reducing hospital admissions including the development of new services and pathways. 
  • Education and Training: Provide guidance and support to nursing staff, GP Registrars, and allied health fostering a culture of continuous learning and professional development.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $122,365 to $129,060 per annum (Pro rata). Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type

Salary Packaging:

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

  • Payment of $15,000 staged over three years is available for Nurses and Midwives who move to Tasmania from interstate and remain employed full time for three years or more, on top of relocation allowance already available.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
