Job description:

Care@home is a state-wide interdisciplinary service that remotely supports patients, to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions.

The Team:

Care@home is a state-wide interdisciplinary service that remotely supports patients, to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions.

This virtual care service has been established as a state-wide navigation hub, that provides a single point of access for home and community-based care; making it easier for both consumers and clinicians to identify what care pathways are available to support people in their preferred environment.

This interdisciplinary service employs General Practitioners who deliver virtual clinical consultations, support for GP Registrars and junior doctors and short-term primary care deployment to community settings where there are critical risks to the delivery of services

The Role:

The Clinical Lead will:

  • Provide a high level of leadership and direction, ensuring resources are efficiently utilised, services are effectively integrated within the service and the highest standards of service delivery are achieved in accordance with hospital/service policy directions, service standards and financial targets.
  • Work collaboratively with the Nursing Director to lead and manage the activities of Care@home in accordance with service policy and administrative instructions as varied from time to time.
  • Have due regard for the professional expertise and special knowledge of clinical staff members of Care@home and will extend to them professional autonomy in so far as that is consistent with the effective management of the stream and of the hospital/service.
  • Provide the Executive Director of Service (EDOS) with high quality and authoritative advice and support.

Details of appointment:

Permanent part time, day worker position, working 38 hours per fortnight, commencing from as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time and casual vacancies.

Salary:

Specialist Medical Practitioner Level 1- Senior Specialist Medical Practitioner Level 3 $210,000 - $310,349 per annum, pro rata + 11.5% superannuation + access to salary packaging.

Please note that access to the Senior Specialist Medical Practitioner salary range $291,472 -$301,349 is subject to the successful applicant satisfying the criteria for Senior Specialist Medical Practitioner Level 1, 2 or 3 as outlined in the Medical Practitioners (Tasmanian State Service) Award

  • Salary range is in accordance with Medical Practitioners Agreement 2022

In addition, the following allowances and benefit, calculated on the salaried incremental point may be available

  • Salary packaging options include a range of benefits like living expenses up to $9,010 and $2,650 in meals and entertainment every year
  • Motor vehicle allowance of $28,389 per annum, pro rata
  • Participation in Private Practice Scheme 35% or allowance in lieu of private practice schemes 35%
  • Continuing Professional Development Allowance of $16,480 per annum, pro rata
  • Professional development and accelerated pathways.
  • A range of leave entitlements, including study leave and Professional Development Support.
  • Fitness Passport – You, and your family can access 40+ fitness facilities across Tasmania for just $13.95 per week per person, or $25.95 for a family.
  • Other allowances as applicable.

Eligibility:

Eligibility: Successful applicants will be required to meet the essential criteria as

  • Specialist or limited registration with the Medical Board of Australia in a relevant specialty

*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.
