Job description:

The Transition Care Program (TCP) team focus on delivering excellence in healthcare and partnering with a range of service providers to achieve improved outcomes for our consumers.

The Role:

Within Tasmanian Health Services framework:

  • Provide Case management services to older persons identified as medically stable ready for discharge from the Acute Care Sector and who would benefit from time limited, goal orientated, therapy focused restorative care, within the Southern Region.
  • Comply with the Department of Health and Ageing Transition Care Program Guidelines, Legislation and Reporting requirements.

Duties:

  • Link, refer and co-ordinate services through care plans
  • Monitor service delivery
  • Collaborate with a multiciliary team to enable a person to meet their goals
  • Case closure and evaluation

The Team:

The Tasmanian Health Service provides the Transition Care Program in Southern Tasmania, that seeks to optimise the functioning and independence of older Tasmanian people after a hospital stay. Transition Care is goal orientated, time limited and therapy focused. The Transition Care Program team focus on delivering excellence in healthcare and partnering with a range of service providers to achieve improved outcomes for our consumers.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $79,529 - $91,517 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Public Sector Unions Wages Agreement 2022
