Job description:

We are seeking a Clinical Nurse Consultant - Hospital Aged Care Liaison Team to provide expert clinical advice and leadership within an interdisciplinary team providing assessment, care coordination and implementation of planned outreach support to older people in the emergency department, community or residential aged care setting to enable care needs to be met in the most appropriate environment. Undertake a broad consultative role by initiating and maintaining a comprehensive care network, building and maintaining links with providers in the primary health setting, including community service providers, General Practitioners, Residential Aged Care Facilities and the Rural Inpatient Facilities. Develop and implement evidence-based practices that promote the efficient and effective provision of patient centred aged care services within the healthcare setting.

The role:

The Hospital Aged Care Liaison Team (HALT):

  • Passionate and dedicated team of specialist nurses focused on supporting the needs of our elderly patients.
  • Have a comprehensive knowledge of community packages, services, and programs available.
  • Work collaboratively with each ward, providing leadership and support with safe and timely discharge planning. 

Details of Appointment

Permanent, full time, day worker position working 76 hours per fortnight, commencing as soon as you can join our team.

  • Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.

Salary: $111,506 - $118,336 per annum. Our Employer 11% superannuation contribution is on top of this amount.

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible. 

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023 and Nurses and Midwives (Tasmanian State Service) Interim Agreement 2013

  1. Registered with the Nursing and Midwifery Board of Australia
