About the Opportunity
Join the Community Rapid Response Service (ComRRS) and play a leading role in delivering advanced clinical care to people across North West Tasmania.
This innovative community-based service provides rapid assessment, treatment and support for people experiencing acute illness, injury, or exacerbations of chronic and complex conditions. By delivering timely care in the community, ComRRS helps people avoid unnecessary hospital presentations and admissions while achieving better health outcomes closer to home.
As a Nurse Practitioner, you'll work with a high degree of clinical autonomy, applying advanced assessment, diagnostic and treatment skills while collaborating with community nursing teams, General Practitioners, pharmacists, allied health professionals and other healthcare providers.
This is an opportunity to practise at the top of your scope, influence service delivery, lead clinical excellence and contribute to the ongoing development of an important hospital avoidance service.

What You'll Do
- Deliver advanced clinical assessment, diagnosis, treatment and ongoing management of clients in community settings.
- Provide rapid and intensive responses to people experiencing acute illness, injury, or deterioration of chronic and complex conditions.
- Initiate and interpret diagnostic investigations and implement evidence-based treatment plans within Nurse Practitioner scope of practice.
- Work collaboratively with General Practitioners, community providers and multidisciplinary teams to optimise patient outcomes.
- Lead clinical excellence through quality improvement, research and evidence-based practice.
- Mentor, educate and support nursing and multidisciplinary staff.
- Contribute to the ongoing development and evaluation of the Community Rapid Response Service.

What We're Looking For
We're seeking an experienced and highly motivated Nurse Practitioner who is passionate about delivering innovative, person-centred healthcare in the community.
You will have:
- Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse and endorsement as a Nurse Practitioner.
- Extensive clinical experience and advanced nursing expertise within a primary health care, community health or similar practice environment.
- Demonstrated expertise in comprehensive assessment, diagnosis and management of acute, chronic and complex conditions.
- Advanced clinical judgement and decision-making skills.
- Strong leadership capabilities with the ability to educate, mentor and influence others.
- Excellent communication and stakeholder engagement skills, including the ability to work effectively with General Practitioners and multidisciplinary teams.
- A commitment to evidence-based practice, quality improvement and achieving exceptional patient outcomes.
- Current Driver's Licence.

Details of Appointment
Multiple FTE available, up to full time
Shift worker position, working as part of the Community Rapid Response Service.
Notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full-time, part-time and casual vacancies.
Salary: $148,470 per annum (pro rata), plus 12% employer superannuation.

Successful applicants will be required to meet the following essential requirements:
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse and endorsed to practise as a Nurse Practitioner.
