Clinical Nurse Specialist
Hellyer Community Nursing and Home Care Services is currently seeking applications for a passionate and experienced Clinical Nurse Specialist to join our dedicated team. This part-time role provides vital clinical support to both nursing and home care teams, across a 7-day per week service, ensuring high-quality care is delivered to clients in the community.
About the Role:
As a Clinical Nurse Specialist, you will play a critical role in supporting hospital avoidance initiatives and managing clients with injuries, illnesses, or exacerbations of chronic and complex health conditions in the community. This role requires strong clinical judgement, leadership, and the ability to work collaboratively across healthcare sectors.
Essential Skills and Attributes:
To be successful in this role, you will bring:
- Clinical Expertise: Demonstrated experience in assessment, care planning, and intervention to support hospital avoidance and community-based care.
- Communication & Collaboration: Excellent interpersonal skills and the ability to coordinate care effectively with hospitals, GPs, community pharmacists, allied health professionals, and clients/families/carers.
- Autonomy & Teamwork: A proven ability to work independently as well as within a multidisciplinary team environment.
- Leadership & Organisation: Strong leadership and organisational skills, with the ability to apply clinical risk management, drive quality improvement initiatives, support nursing research and education, and uphold professional practice standards.
- Commitment to Learning: A passion for promoting a learning culture through education and clinical decision-making support, along with a dedication to ongoing professional development.
The Team:
Hellyer Community Nursing and Home Care Service (HCHN&HCS) operates within a Primary Health framework to deliver nursing and home care services to clients within our catchment area. Our catchment covers from Sisters Hills in the west, to West Ulverstone in the East and south to Waratah. Clients also receive care in Community Nursing clinics in Burnie and Wynyard by appointment only.
The Community Nursing Services operating from HLCN&HCS include the following:
- Nursing
- Hospital in the Home
- Palliative Care
- Personal Care
- Domestic Assistance
- Home Maintenance
The primary goal of the Community Services is to assist with the below:
• Facilitate hospital avoidance
• Facilitate discharge from hospital
• Support people at risk to live at home for as long as possible
The Hellyer Community Nursing (HCHN) delivery models include:
• Community Nursing
• Community Nursing Enhanced Care Service (CoNECS)
• Hospital in the Home (HITH) programme
Nursing care provided by HCHN includes but is not limited to:
• Chronic and complex wound care including Negative Pressure Wound Therapy
• Management of venous access devices (PIVC & CVAD)
• Indwelling and suprapubic catheter management
• End of life palliative care including syringe driver management
• Post-acute care following discharge from hospital
• Medication management including support of medication administration via various routes and devices.
Details of appointment:
Permanent part time, shift worker position, working 56 hours per fortnight, commencing from a mutually agreed date. *notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
- Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
In addition, the following allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10 % based on qualification type
Eligibility:
Successful applicants will be required to meet the essential criteria.
- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
- Current Driver’s Licence
Applicants should note the following criteria are desirable:
- Post Graduate qualification in Community Nursing or working towards a relevant postgraduate qualification
