Job description:

We are seeking a Clinical Nurse Specialist - Aged Care Assessment Service Assessor to be part of a dynamic multidisciplinary team working across North West coast of Tasmania.

The Team:

The Aged Care Assessment Team (ACAS) is a dynamic multidisciplinary team working across North West coast of Tasmania.  Under the My Aged Care, Single Assessment System the team is responsible for assessing the needs of eligible community members for services and support to maintain their independence and wellbeing using a wellness and reablement approach.

The Role:

The Clinical Nurse Specialist (CNS) - Aged Care Assessment Service (ACAS) Assessor:

  • Conducts clinical and non-clinical assessments of the restorative, physical, medical, psychological, cultural, and social needs of people referred to the ACAS, which includes liaising and consulting with clients, other ACAS members, medical personnel, carers/family and appropriate service providers to identify appropriate care options.
  • Promote the role of ACAS, providing community education and increased awareness of services available to assist target groups. 
  • Participates in Aged Care Assessment Program (ACAP) data collection processes, for evaluation and research purposes. 
  • Screens incoming referrals and determines appropriateness for ACAS, prioritising according to National codes. 

You will need:

  • You will be an excellent communicator, you will be able to liaise with clients, their supporters and other professionals to ensure the needs of the individual are met.
  • You will have sound written and verbal communication skills. 
  • You will be able to meet team and organisational Key Performance Indicators.

Details of appointment:

Permanent, part time, day worker position, working 45.5 hours per fortnight, commencing from a mutually agreed date.

*notwithstanding hours to be negotiated with the successful applicant.

Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
  • Current Driver’s Licence

Applicants should note the following criteria are desirable:

  • Postgraduate qualifications or proven experience in the assessment and management of clients with dementia
